Case Management is a collaborative process of assessment, planning, facilitation and advocacy for options and services to meet an individual’s health needs. The Complex Case Management service provides support and advice to Cabrini Health patients and their families who have complex issues. This involves contact across your inpatient stay through to discharge by organising any follow up services which may be required in the community to support you (such as home help and meals), or by facilitating transfer to rehabilitation or residential aged care facilities for those who are no longer able to live safely in the community.
As required the Case Manager will also remain in contact with you following your discharge home to see how you are managing and to offer any additional supports which may be required.
