Why is this important?
Along with improved functional ability, rehabilitation programs aim to support patients to return to either their previous, type of accommodation or an improved type of accommodation. This is largely determined by the patient’s ability to care for themselves; the support available in their home and any additional conditions or diseases they patient might have.

In the most recent period, 95% of patients returned to the same level of accommodation following the end of their rehabilitation program.
Note: The predominant age group for patients at Cabrini Elsternwick is 75 to 95 years old. These patients present with multiple co-morbidities when admitted for rehabilitation.
