As Heart Health Week approaches, Cabrini Hospital proudly announces the ground-breaking impact of CARDIAPP, a cutting-edge digital education tool that is revolutionising cardiac rehabilitation. This innovative platform has been instrumental in supporting hundreds of heart patients through their journey to recovery, all from the comfort and convenience of their own homes.

Michelle Comas, a distinguished Co-ordinator of Cardiac and Pulmonary Rehabilitation at Cabrini, emphasised the transformative nature of CARDIAPP, stating, “CARDIAPP is a game-changer, significantly improving how heart patients manage their condition and enhancing their overall quality of life.”

Developed with meticulous attention to detail, CARDIAPP bridges the gap between hospital care and at-home support, eliminating the need for frequent hospital visits.

Peter Rady, Director of Operations at Medicity, a division of the Cabrini Technology Group, reports, “Since its launch in October last year, Cabrini Cardiac Rehab patients have utilised 1,853 CARDIAPP content articles, enhancing their path to recovery.” These articles provide up-to-date information and personalised guidance on managing risk factors, crafting individual exercise routines, dietary recommendations, and more, all facilitated by Cabrini’s esteemed multidisciplinary Cardiac Rehabilitation team.

CARDIAPP, a program featuring cardiology educational materials encompassing text, images, and videos, is generated using Medicity’s eCaptis Patient Journey Digital Platform. Moreover, CARDIAPP empowers patients to take control of their health by helping them understand vital metrics such as cholesterol levels, blood pressure, and blood sugar levels. Armed with this knowledge, patients are motivated to adopt positive lifestyle changes, thereby enhancing their overall well-being.

Based on feedback from patients, Michael, aged 56, finds CARDIAPP highly beneficial, emphasising its wealth of valuable information and informative videos. Patricia, aged 80, similarly praises CARDIAPP, describing it as excellent, easy to navigate, and thoroughly enjoyable.

Representing a significant leap forward in cardiac rehabilitation, CARDIAPP offers patients diagnosed with cardiovascular diseases or recent hospital admissions unparalleled access to essential resources and support, all without the constraints of hospital visits.

Megan Potter, Director of Cabrini Foundation, highlighted the impact that Cabrini donors have on improving health outcomes and enhancing patient experiences. “Through initiatives like CARDIAPP, Foundation donors and partners are helping ensure that all patients receive the very best care, regardless of their geographical location,” Ms Potter said.

The statistics from the Heart Foundation underscore the urgent need for innovative solutions in cardiac care, with one person being hospitalised due to a heart attack every nine minutes.
